Maternity Care Coalition is Hiring a HFA Program Coordinator Near Philadelphia, PA

Organizational Summary
In 1980, a group of Philadelphia health and social justice activists came together over concerns about the well-being of pregnant people and infants. They created Maternity Care Coalition (MCC) and advocated for change with policymakers and decision makers.
Today, we continue to center children and their families by supporting, referring, educating, researching, and advocating.

Position Summary
Responsible for supporting the Healthy Families America (HFA) evidence-based, home visiting model with conducting culturally inclusive, trauma informed, psychosocial assessments for newly enrolled pregnant and parenting families across the six county HFA service area, assisting with staff recruitment, facilitating staff trainings, and supporting department quality assurance practices. This position requires in-person and remote work.

Essential Functions
  • Conducts initial psychosocial assessment, Families Resilience & Opportunities for Growth (FROG), for HFA families during in-person home visits and virtually.
  • Supports families with planning for their own health and social service needs by assisting families in identifying goals.
  • Assists with developing service plans by providing service delivery recommendations for pregnant and newly parenting families.
  • Provides referrals to families needing more intensive follow-up for additional services within MCC and externally.
  • Facilitates FROG training using approved HFA model materials to support staff learning and development.
  • Provides individualized FROG training support to new HFA staff through home visit shadows (virtual and in-person).
  • Keeps careful records of all family contacts made and follows up as necessary
  • Conducts quality assurance processes including file reviews, family satisfaction calls, referral completion, and other practices as identified by the HFA Director(s).
  • Participates in regular reflective supervision to report on and discuss work.
  • Ensures confidentiality of family information, sharing pertinent information only with a family's approval.
  • Supports with recruitment and hiring staff.
  • Provides input on department training needs and annual training plans.
  • Strengthens own and staff’s skills through personal and professional development

Other Tasks
  • Adheres to the MCC’s security guidelines and ensures the appropriate handling of sensitive information.
  • Attends relevant staff meetings to promote communication and execution of goals.
  • Identifies and maintains regular contact with appropriate social service and community agencies to facilitate referrals and resources for families.
  • Monitors efforts to meet program deliverables
  • Assumes duties of supervisor as necessary and appropriate.
  • Other duties as assigned within the scope of position expectations.

Knowledge, Skills, and Abilities
  • Understanding of MCC’s theory of change, mission, goals, and objectives and ability to work independently with a high level of energy and contribute as part of a larger team.
  • Knowledge of the concepts and practices associated with community health issues, particularly with pregnant women, prenatal care, parenting and other services for mothers, young children, and their families.
  • Ability to travel in and around the Greater Philadelphia region to conduct home visits and community outreach.
  • Knowledge of and sensitivity to systemic barriers impacting the community.
  • Experience in assessing needs, identifying resources, making referrals, following up, and documenting interactions.
  • Demonstrated clinical skills.
  • Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, community partners, families, and members of the community.
  • Ability to operate a computer and use common software programs including Microsoft Office and customized databases.
  • Effective and accurate documentation of case notes and family data according to established documentation procedures.
  • Ability to organize tasks in an efficient manner and follow-up and follow-through with strong attention to detail.

Experience, Education, and Licensure
  • Bachelor's degree in Social Work, Psychology, Public Health or related field required
  • Valid driver’s license with a clean record and driving experience required
  • 2 years’ experience providing direct services in a community setting required
  • Experience with conducting in-depth assessments required
  • Demonstrated strong organizational and administrative skills required
  • Knowledge of maternal and child health issues encouraged to apply
  • Demonstrated experience of trauma-informed practice encouraged to apply
  • Bilingual encouraged to apply (Spanish, Arabic, French)
  • This position requires in person and remote work
  • Nontraditional hours required at times to meet client family needs
