PHMC is Hiring a Program Coordinator Near Philadelphia, PA

JOB OVERVIEW:

The Program Coordinator will oversee the activities of the Family Planning Access Navigator (FPAN) program, including outreach/recruitment, intake, and case navigation services. Primary responsibilities include coordinating program activities, attending community outreach events, making initial contacts with potential clients, conducting intakes with clients, supporting the development and implementation of project activities and deliverables, direct services to clients, and general support. This position will have both internal project coordination responsibilities and outward-facing responsibilities. As such, this position requires strong project and timeline management skills, as well as strong oral and written communication skills. This position will require travel throughout Philadelphia. Some evening or weekend work is required.

RESPONSIBILITIES:

  • Coordinate program activities, including data management and coordination of navigation activities.
  • Maintain first point of contact with referral sources and potential clients to services.
  • Identify and outreach to community organizations and businesses in targeted communities.
  • Provide community education about services available through Title X Family Planning health care funds.
  • Maintain expertise in Title X provider offerings and services.
  • Coordinate intakes with clients.
  • Maintain a calendar for intakes in order to streamline processes and enhance efficiency to complete intakes with referred clients.
  • Provide potential new patients with accurate information about the clinical providers and help to decide which provider they wish to access.
  • Provide referrals and linkages for participants to alleviate barriers to services, including health insurance, education, employment, mental and physical health, and various other needs.
  • Posting on internal social media pages once a week.
  • Interacting/engaging with organizations to connect with potential clients.
  • Maintain awareness for community events to attend
  • Maintain liaisons with clinical providers and other Title X navigation programs.
  • Serve as liaison between community contacts and partners and the HPC management team, providing updates on an ongoing basis in written and verbal formats.
  • Work with staff and other teams to develop recruitment and referral strategies and assist supervisor with the development of a plan to meet targets.
  • Assist with data tracking and collection. Maintain up-to-date tracking in the database.
  • Give presentations on the program, as requested, to community groups, school boards, and funders.
  • Maintain ongoing knowledge of the rules and regulations of public health programs. Maintain knowledge of federally qualified health centers and patient assistance programs. Make referrals to such programs as needed.
  • Refer uninsured and underinsured individuals for insurance counseling and enrollment in the ACA Marketplace.
  • Attend all required meetings and trainings.
  • Carry an ongoing caseload of clients who need additional supports to maintain their reproductive health.
  • Where appropriate, provide intensive supports to individuals who need ongoing education and linkage to Title X and other services.
  • Work as part of a team. Function in a peer-to-peer learning environment.
  • Work with the program team to assist in the delivery of other program activities.

Administrative Activities:

  • Support development and submission of program reports and other written documentation as required.
  • Work collaboratively between other programs in HPC and other agencies/organizations.
  • Communicate formally on a biweekly or weekly basis with supervisor.
  • Attend local and regional meetings and trainings as needed.
  • Complete all other responsibilities as assigned.

SKILLS:

  • Excellent oral and written communication skills, especially in communicating with clients
  • Strong youth engagement and positive youth development skills
  • Ability to handle multiple responsibilities and to work independently
  • Familiarity with Philadelphia social service organizations
  • Strong interpersonal skills
  • Superior cultural competence related to race, gender, and sexual orientation
  • Knowledge of issues related to positive youth development, child development, and family life skills
  • Comfortable working with youth in low-income communities
  • Willingness to work a flexible schedule, including some evening/weekend hours as requested
  • Ability to successfully pass child abuse clearance and HIPAA certification
  • Proficiency with Microsoft Office and web-based data collection applications
  • Willingness to travel throughout Philadelphia
  • Bilingual (Spanish) is a plus.

EDUCATION & EXPERIENCE REQUIREMENTS:

  • Bachelor’s degree in social work, child development, psychology, or a related field required
  • At least two years of direct service experience working with youth
  • Experience working with LGBTQ youth is a plus.
  • At least one year of program coordination experience preferred
  • Previous community engagement work preferred
