The Educational Alliance is Hiring a Case Manager Near New York, NY

The Weinberg Center for Balanced Living (WCBL), an innovative Older Adult Center funded by the New York City Department of the Aging (NYC Aging), is integrated into MCC. WCBL aims to address the varied needs, abilities, and interests of New Yorkers ages 60 . WCBL’s holistic wellness programs make it a premier destination for older New Yorkers to enhance their physical and nutritional health, artistic prowess, and cultural and social connections and to receive social services support that allow them to age in place with dignity. WCBL serves an older adult population whose primary languages include English, Chinese, Spanish, and Russian.

About the Role

Provide case assessment, information and referral, assistance, and case management to clients and caregivers who participate in the Weinberg Center for Balanced Living (WCBL), and/or reside at Podell Houses or Weinberg Apartments. This position requires on-site presence at the Manny Cantor Center four days per week and one day per week at the Weinberg Apartments. Services will be provided in both English and Cantonese/Mandarin.

  • Regular contact with clients, developing individualized care plans that are culturally sensitive and language appropriate
  • Work as member of interdisciplinary team to support WCBL program activities, including facilitating at least one group session for members and assisting with special events
  • Participate in regularly scheduled case conferences, weekly supervision and WCBL staff meetings, and other special agency staff meetings
  • Direct contact with the Building Manager of Podell Houses and Weinberg Apartments
  • Direct contact with NYC Aging Program Officer during social services audits

Key Responsibilities

Case Assistance and Management

  • Maintain a regular caseload of clients, assess clients for social work services, conduct home visits, and provide case assistance and management services to clients and their caregivers
  • Assist with obtaining concrete services, including but not limited to completing clients’ enrollment/re-enrollment applications for 202 housing and benefits and entitlements programs, and provide crisis intervention when needed
  • Develop individualized care plans that ensure a seamless system of services and are culturally sensitive and language appropriate
  • Refer clients to external organizations and community partners and share information about community resources with WCBL members, clients, and colleagues
  • Assist older adults with WCBL membership registration and renewal applications and annually update client’s membership profile on NYC Aging’s online database
  • Maintain appropriate physical and digital client files and documentation, such as consent forms, case logs, and information and referral logs, for monthly submission to NYC Aging
  • Within 4 days of any service provided to a client, input clear and comprehensive case notes into NYC Aging’s online database, following the funder’s prescribed case note format
  • Escort clients to appointments for benefits and entitlement meetings (e.g., Social Security Administration, Medicaid, and other offices) when appropriate
  • Provide interpretation services and assist in translation of written communication as needed

Program Support

  • Assist in coordination of special events at 202 housing and at WCBL
  • Facilitate groups that stimulate client support and engagement and collaborate with community providers and colleagues to implement educational workshops about relevant social services topics (e.g., housing; entitlements; end-of-life planning; grief support; etc.)
  • Respond to telephone, email and in-person inquiries regarding WCBL activities 
  • Assist with unexpected disruptions in program activities, such as staff absences, weather emergencies, and provide support to daily programs, as directed by Program Directors

Job Requirements and Desired Skills

  • BSW/BA with at least 1 year experience working with older adults, especially those who may live with age-related cognitive and mental impairments
  • Bilingual Chinese/English required
  • Knowledge of entitlements and community-based resources for older adults
  • Ability to demonstrate excellent judgement and discretion in dealing with confidential and sensitive information
  • Skilled in client need assessment, prioritization of case load needs, and crisis intervention
  • Demonstrated knowledge of community resources for individuals and families
  • Ability to appropriately interact with New Yorkers of diverse religious, cultural and social backgrounds required.
  • Excellent interpersonal and written and verbal communication skills required.
  • Ability to handle challenging situations in a professional and courteous manner
  • Technological proficiency with Microsoft Suite, Google Suite, and internet research
  • Maintenance of confidentiality of records
  • Ability to operate in a deadline-driven environment
