CENTER FOR URBAN COMMUNITY SVC is Hiring a Case Manager Near New York, NY

Description
The Center for Urban Community Services, (CUCS), is an acclaimed human services organization and one of the nation's largest providers of housing and social services. CUCS is a leader in implementing new practices, sharing knowledge and assisting in shaping local, state and national strategies, so that persons who are homeless, low-income, living with mental illness, or have other special needs can live successfully in the community.
This position works within the Manhattan Outreach Consortium (MOC), specifically with the CUCS Street Outreach Team, and works collaboratively with Janian Medical and Psychiatry program and providers.
  • Responsible for a caseload of medically and psychiatrically vulnerable clients.
  • Engage with all Outreach clients and prospective clients as needed around medical and psychiatric needs.
  • Responsible for escorting clients assist with their medical or psychiatric needs as assigned by supervisor.
  • Complete progress notes according to agency and funding source guidelines regarding timeliness.
  • Complete all other service documentation according to agency and funding source guidelines regarding timeliness.
  • Provide documentation that is accurate, relevant, concise, appropriate to the setting, and supportive of quality services.
  • Refer clients to housing opportunities; Assist with interview preparation, apartment viewings, and completing all paperwork for housing applications.
  • Make sure that Janian Providers are aware that any client seeing Janian is interviewing for perm housing and that they are part of decision making around what is appropriate placement for client. Partner with Janian to connect to providers at new program or connect with providers in the community as necessary
  • Anticipate, prevent, and respond to crises, by working with the client and other staff members to identify and resolve factors that may lead to a crisis.
  • Recognizing as early as possible when a client is in crisis or moving into one and intervening as rapidly as possible to address the situation, including working to join with the client in identifying and addressing it and engaging other supports as necessary.
  • Review referrals before sending to psych/SMT, before psych/SMT visit, and ensure client is aware of referral, reminded of appointments, coordinate and/or provide escort when needed, cancel and reschedule appointment if client cannot attend, provide client with follow up items from appointment such as obtaining medication and providing information about attending subsequent appointments. Consult with SMT to confirm first appointment was scheduled.
  • Escort clients to medical appointments, labs and diagnostic imaging centers, as needed.
  • Collaborate with Healthcare Navigator around utilizing E-PACES to determine client medical coverage when necessary.
  • Track clients' Medicaid/Medicare certification to ensure recertification and prevent lapses in coverage in collaboration with Case Managers.
  • Assist SMT with coordination of special projects and public health initiatives such as vaccinations.
  • On days psychiatry and Street Medicine Team (SMT) providers are with MOC team:
    • In-person sign in and sign out with SMT and psychiatric providers.
    • Ensure Case Managers submit referrals.
    • Communicate SMT and psychiatric sign out to team at end of day (including recommendations and action items)
    • Assist in looking for clients or escorting clients to team office for scheduled appointments
    • Spend time in the field (either with SMT providers or independently) locating and engaging clients
    • Meet monthly with team leadership and SMT providers to review and update medically vulnerable list
    • Manage psychiatric and SMT schedule
  • Collaborating with all other CUCS Departments and external Agency as required
  • Monthly meeting of team liaisons
Requirements
Experience and Education Required:
  • Must have valid driver's license.
  • Relevant experience working with population served by the program preferred.
  • Applicants who speak Spanish will be prioritized
  • This is a field base position, must be able to travel
  • CM 1: High school diploma and 2 years experience
  • CM 2: Bachelors degree or HS diploma or equivalent and 4 years of relevant work experience
  • CM 3: BSW and 1 year relevant experience (excluding fieldwork) or Bachelors degree and 2 years relevant experience or HS diploma and 6 years relevant experience
  • For applicants without college degrees, every 30 credits can be substituted for 1 year of experience
  • Demonstrated ability to provide services to a specialized population, computer literacy and good written and verbal communication skills are required.
  • Must be able to work effectively as part of a multidisciplinary team
  • 4 Overnight shifts are required per year for Quarterly Homeless Outreach Counts
