JOB SUMMARY
The Case Manager is part of a human service team that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development. The Case Manager plays a key role in executing and updating service and care plans, coordinating direct services and service referrals to external providers, and monitoring resident behaviors and practices.
Pay: $23.07 an hour- (approx. $47,900 annual)
FSLA: Non-exempt. Overtime is available.
Location: Midtown Manhattan, NY
Schedule: Tuesday-Saturday either 9am-5pm or 10am-6pm
These duties are essential and specific to the successful implementation of this position.

GENERAL DUTIES
ENTRY LEVEL QUALIFICATIONS AND REQUIREMENTS
Education and Experience: High School Diploma or GED required. Bachelor’s Degree in social work or psychology preferred. Three years of experience working in a shelter or similar field can be substituted for the education requirement.
