Griffith Centers is Hiring an Adult Case Manager Near Colorado, CO
- Establish rapport to have an effective working relationship with individuals/families/guardians.
- Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team.
- Develop, monitor, and document mandated services and programming, and ensure client satisfaction. Ensure individual files are audit ready.
- Conduct home visits, monitor, and support host homes. Provide technical support.
- Participate in rotation for “on call” to support our individuals.
- Support individuals at Interdisciplinary team (IDT Team) meetings and advocate as appropriate.
- In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Griffith Centers PASA supervisors and collaborating agencies.
- Participate in ongoing program evaluation to ensure program effectiveness.
- Bachelor's degree with one year experience.
- QIDP certification (Qualified Intellectual Disability Professional) Preferred.
- Experience with I/DD HCPF rules and regulations.
- Experience with maximizing funding benefits for individual and program needs.
- Valid Regular Driver’s License, with acceptable driving record.
- CPR&1st Aid or ability to obtain within 30 days of hire.
- Safety Care or ability to obtain within 30 days of hire.
- QMAP or ability to obtain within 30 days of hire.
