Pay: $17.00 per hour

Work Schedule: Monday - Friday 8am to 4:30pm or 9am to 5:30pm

OBJECTIVE:

The Programs Case Manager II assists in the design and delivery of program activities for person with mental, physical, and/or intellectual/developmental disabilities. This position will be responsible for continuous support to the Employment and Community based programs. This role serves as a champion and model of Goodwill’s values.

EDUCATION AND EXPERIENCE: 

Applicable field or equivalent combination of training and experience in a social service, counseling or rehabilitation setting, providing direct services to persons with physical, emotional and/or developmental disabilities will also be considered. A valid Colorado driver’s license, vehicle insurance, and willingness to use personal vehicle for community work is required.

OTHER REQUIREMENTS/PREFERENCES: 

It is preferred that the incumbent have excellent written and verbal communication skills and demonstrate the ability to interact with various types of internal and external customers. It is also preferred that the incumbent have the ability to relate effectively to people with disabilities, their parents, host home providers and guardians and be empathetic toward their needs. The incumbent must have the ability to work independently, cooperatively and to exercise effective planning. The Case Manager II, Programs must demonstrate flexibility within the framework and pressure of established deadlines. The incumbent must be able to write clear, concise and accurate reports and plans, submit them in a timely fashion and possess excellent time management skills. The incumbent must have a working knowledge of community resources, have the ability to program, train and be creative in motivating Participants toward individual success. The incumbent must also conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation.

Overtime may be required as well as some travel to attend meetings and required training. The ability to work occasional weekends and nights for the Residential on-call system is required. 

Per contract requirements with vehicle insurance provider: Qualified candidates must be 21 years of age and have no major traffic violations for previous 3 years (for driving/insurance purposes). Must have reliable transportation; must also be able to operate company vehicle.

KEY RESPONSIBILITY AREAS:

The Case Manager II, Programs will perform the following duties personally or through delegation:

• Supervises a caseload of consumers consisting of all disability areas and encourages the utilization of multiple community agencies serving persons with mental, physical, and/or intellectual/developmental disabilities. 

• Develops and maintains contact with other agency staff and local service providers to develop specialized services for consumers and their families.

• Assigns, guides, coaches, and trains consumers and providers in alignment with Community Employment and Residential, and Goodwill policy and procedures. Provides information concerning case management practices, administrative rules and regulations, community resources, vendors, and programs.

• Serves as a resource to address sensitive consumer needs and/or accommodations.

• Develops Individual Services and Supports Plans for each consumer within the programs, implements and monitors plan progress. Adjusts and modifies development plan as needed to meet changing consumer needs.

• Developed Person Centered Assessments, as well as Health and Safety Assessments. 

• Complete necessary financial paperwork for consumers.

• Participates on the inter-disciplinary team to review each consumer’s case and participates in all inter-disciplinary team meetings for each consumer.

• Conduct home visits and community visits with consumers and providers. 

• Prepares or dictates case progress notes, social summaries, and other reports concerning the consumer’s case.

• Attends staff meetings concerning new or revised policies and procedures.

• Collaborates with and receives direct supervision from Manager in making decisions related to consumer services.

• Participate in Residential’s on-call system and be seen as a expert in handling consumer incident reports during non-work routine hours. 

• Performs any other duties as required or assigned.

**Criminal Background Check:**

Pursuant to C.R.S. §27-90-111 and CDHS Policy VI-2.4, any independent contractor, and its agent(s), who is designated by the Executive Director or the Executive Director's designee to be a contracting employee under C.R.S. §27-90-111, who has direct contact with vulnerable persons in a state-operated facility, or who provides state-funded services that involve direct contact with vulnerable persons in the vulnerable person's home or residence, shall: 

  1. submit to a criminal background check, and
  2. report any arrests, charges, or summonses for any disqualifying offense as specified by C.R.S. §27-90-111 to the State.

Any Contractor or its agent(s), who does not comply with C.R.S. §27-90-111 and DHS Policy VI-2.4, may, at the sole discretion of the State, be suspended or terminated.
