We are looking for dedicated Licensed Practical Nurses that would provide amazing home health care to all their patients in West side of Maricopa County.

Full-Time:

We offer Hybrid Pay Rate with a base pay of $17.50 per hour plus additional $27.00 per visit the Full IRS of $0.67 a mile - mileage reimbursement (this will be discussed during the interview.)

Per Diem:

We offer $50.00 per visit the Full IRS of $0.67 a mile (mileage reimbursement)

** Must have at least 6 months of experience in home health care. **

We provide 3-5 days of paid training for Full-Time

A Licensed Practical Nurse License in the state of Arizona or with Multistate, AZ Fingerprint Clearance Card, CPR/BLS Certification and current Negative TB Test are required for the position.

• Reliable vehicle
• Driver's License and Auto Insurance
• Covid19 Vaccination Record / Signed Religious Exemption Form
• Flu Shot/signed Declination form
• Hep B test result/Signed Declination Form
• Immunizations (TDaP, MMR, Varicella)
