AREA COVERAGE:

• Central: Phoenix, Scottsdale, Tempe Mesa, Glendale

SCHEDULE:

• 40 hours a week for 5 days for 5 days
• 8:00 AM to 5:00 PM

ABOUT THE POSITION:

The Licensed Practical Nurse or Licensed Vocational Nurse provides nursing care to patients in the home as prescribed by the physician and in compliance with applicable laws and regulations and the policies of the company.

• Participates in coordination of home health services, appropriately reporting the need for disciplines (CNA, OT, PT, MSW, etc.) to the case manager and/or clinical supervisor.
• Implements plan of care initiated by the case manager.

• Monitors patients’ condition including fluid intake and output and compose patient charts
• Performs other duties as assigned by the RN such as but not limited to: administer injections, prescribed medications, enemas, etc.; Clean and dress wounds and assist with other basic medical care tasks; Collects samples for lab testing as assigned.
• Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance and adhere to the company policies and protocols.
• Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.

Requirements:

• PN License in the State of Arizona
• AZ Fingerprint clearance card
• Updated Credentials

  2. Negative TB Test Result
  3. Driver's License and Auto Insurance
  4. Flu Shot / Signed Declination Form
  5. Hep B / Signed Declination Form
• Covid-19 Vaccination / Signed Religious Exemption Form
• Immunization Records (MMR, Tdap, Varicella)
• For Per Diem: with at least 6 months of experience in Home Health

Others:

• Will be driving to patient's residences
• Must be able to lift 25 lbs
• Must be in good health and be able to fulfill the physical requirement
• Must be willing to drive all-over (Nevada**)
• With Home Health Care experience preferred
• W4 type of employment
