We are seeking a Licensed Practical Nurse Lead to join our 7^th Ave Outpatient Clinic. This location provides mental health treatments, including medication monitoring and assistance for persons with addictions.

The Licensed Practical Nurse will work in-office Monday-Friday, standard business hours (no evenings, weekends or holidays).

Job Preview at a Glance:

The Licensed Practical Nurse serves as a central support within the interdisciplinary sites team. This staff member is responsible for providing methadone-dosing services to appropriate clients.

Responsibilities: 

• Completes blood draws and sends blood to the lab
• Monitor known health conditions such as diabetes, high blood pressure, and obesity
• Performs injections
• Review the effects and side effects of medications
• Provide assessment and monitoring of physical/medical status
• Accepts medication orders from physician via telephone and medication records
• Participate in Clinical case staffing
• Facilitate coordination with the consumer’s clinical team and PCP

• Provides accurate documentation of contact with members

Qualifications:

• Licensed in the State of Arizona as an RN
• Ability to obtain fingerprint clearance card
