- Drives to prospective patients’ residences.
- Ensures quality and safe delivery of home health services, including education and information to patients, families and caregivers pertinent to diagnosis and plan of care.
- Implements plan of care initiated by the case manager
- Provides accurate and timely documentation consistent with the plan of care
- Monitors patients’ condition including fluid intake and output, and compose patient charts.
- Assist ailing patients in daily necessary activities such as eating, bathing, etc.
- Administer injections, prescribed medications, enemas, etc.
- Clean and dress wounds, and assist with other basic medical care tasks.
- Collects samples for lab testing as assigned.
- Participates in coordination of home health services, appropriately reporting the need for disciplines (CNA, OT, PT, MSW, etc. ) to the case manager and/or clinical supervisor.
- Uses equipment and supplies effectively and efficiently.
- Attends case conferences and training as required by the agency
- Performs other duties as assigned by the RN.
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent
- Hold a current Licensed Practical Nurse (LPN) in the State of Nevada.
- Graduate of an accredited practical nurse or a vocational nursing program
- At least 6 months of experience in Home health care is preferred.
- Experience in wound care, vacs care, ostomy care, and IV administration is preferred.
- CPR certification.
- Must have a valid driver’s license, and auto insurance.
- Required to present a negative TB test and other tests as required by agency policy
- Must be able to do fieldwork
- Must have excellent written and verbal skills
- Excellent planning, organization, and presentation skills
- Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
- Must be in good health and be able to fulfill the physical requirement of the job.
- Must be able to lift a minimum of 25lbs.
