Director of Nursing (DON)- Hospice

The ideal candidate is a servant leader who has the talent to manage our Hospice Clinical Operations. As the DON, you are the Clinical Expert and Leader who will provide leadership and guidance to all nursing staff, including hiring, training, and performance evaluations. Also, you will monitor and evaluate the effectiveness of the Clinical System and make necessary updates.

DON Responsibilities:
- Oversee and manage the clinical department, ensuring the delivery of optimum patient care.
- Lead Clinical Teams and Clinical Operations
- Monitors and analyzes QAPI and directs clinical staff on agency policy and procedures
- Staff, Develop, and Lead strong Clinical Teams
- Conduct initial assessments of patients' conditions and develop individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor patients' vital signs and symptoms, and report any changes or concerns to the healthcare team
- Provide emotional support and counseling to patients and their families
- Educate patients and families on end-of-life care options and available resources
- Collaborate with interdisciplinary healthcare team members to ensure comprehensive care for patients
- Maintain accurate and up-to-date medical records
- Adhere to HIPAA regulations and maintain patient confidentiality
Requirements:
- Bachelor's degree in Nursing (BSN) or ADN.
- Valid nursing license in (TX, Compact License)
- Minimum [2] years of experience in hospice or palliative care
- Knowledge of Medicare regulations related to hospice care
- Familiarity with nursing home regulations and resident rights
- Strong understanding of acute care practices and procedures
- Excellent knowledge of human anatomy, physiology, and medical terminology
- Demonstrated leadership skills and ability to work effectively in a team environment
- Compassionate nature with the ability to provide emotional support to patients and families
- Excellent communication skills, both verbal and written
