SUMMARY
In conjunction with the Wellness Director, the Assistant Wellness Director is responsible for ensuring Sonata Senior Living Resident Care and Quality Assurance policies and procedures are properly communicated, implemented and executed in an efficient and timely manner. The Assistant Wellness Director assists with ensuring all nursing and resident care practices are carried out in accordance within State guidelines and Company’s quality standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Supervises and participates in all aspects of medication administration and/or assistance with of self-administration of medication preserving safety, dignity, and self-respect of all residents at all the time
- Orders medication(s) as prescribed, following proper medication protocol is followed according to State guidelines.
- Medication Cart daily monitoring to ensure compliance
- Notifies Wellness Director, nursing staff, family members and/or resident and pharmacy when changes in prescription are made.
- Process Medications in ALIS daily and/or as needed to monitor the medication/treatment records for accuracy and completeness
- Monitor ALIS Med Pass, Med Exceptions, PRN medication reports. Report any discrepancies to the Wellness Director
- Monitors and record any changes in the health status of the resident as reported by family, third party providers, residents, caregivers or others. All status changes are documented in the communication log, ALIS observation notes, and communicated to the Wellness Director and team.
- Under the guidance of the Wellness Director, the Assistant Wellness Director will notify the resident and/or the resident’s responsible party (if applicable) and the resident’s physician regarding any change of condition, per Sonata policies and procedures and as necessary.
- Monitor the DARE report, care alerts, 24 Hour Report, neighborhood binders and acuity documentation for accuracy and follow up.
- Assists the Wellness Director with written and verbal communication regarding the provision of third-party services.
- Assists with service plans and assessments, service plans, and other documentation as necessary.
- Responds to emergencies per Sonata policy and procedures
- Provide oversight of Lead Wellness Coordinator including emergency pack audits, transportation scheduling, scheduling, and other duties as assigned
- Notifies the Wellness Director of equipment and supply needs while maintaining the Wellness budget
- Contributes to maintaining survey readiness and ensures compliance with State regulations
- Maintains clear communication with Resident Assistants to monitor timely and appropriate delivery of care is effective
- In the Wellness Director’s absence, the Assistant Wellness Director, will assume the responsibilities of oversight of the day to day operations of the Wellness Department and report directly to the Executive Director.

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Prior to presence in the community, applicant must have completed a Post Hire Health Questionnaire upon offer of employment. A TB test result must be received with 7 days of employment. Previous TB test results cannot be older than 6 months; chest x-ray results 2 years. A Criminal Record Clearance is required prior to team member’s starting date.
EDUCATION and/or EXPERIENCE
Must be 18 years of age or older and have high school degree or the equivalent. Must have and maintain a current First Aid certificate. Training and experience in Certified Nurse’s Aide procedures are desirable and one year or more work experience in home care or skilled nursing for the elderly is preferred. Must have completed Health Screening Report and a Criminal Record Clearance.
Must be licensed as a practical nurse (LPN) in the community’s state. One year or more work experience in home health care, skilled nursing for the elderly or assisted living. Must have completed a Health Screening Report and a Criminal Record Clearance.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or team members of organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
Training and experience in Certified Nursing Aide and/ or licensed as a practitioner nurse (LPN). Must carry and maintain current first aide certificate.
OTHER SKILLS AND ABILITIES
Proficient with MSWord, MS Excel, MS PowerPoint, MS Outlook, and other systems knowledge is helpful. Basic typing skills essential.
PHYSICAL DEMANDS
Position requires the ability to stand, reach, bend/stoop/crouch, push/pull/lift/carry up to 50lbs, twisting/ turning and repetitive motion
