NTSOC is Hiring a Case Manager Near Colorado, CO
- Informs clients about the C-HCBS Medicaid Waiver program and benefits under this waiver. Recruits, manages, develops and leads families through needs and wants. Addresses client’s issues as they arise.
- Ensures compliance with all requirements of the program.
- Stays abreast of regulations and legislation pertaining to the program. Analyzes and implements changes as necessary. Communicates changes with clients as needed.
- Follows policies and standard operation procedures (SOPs) for Medicaid Waiver Programs: Children’s Home and Community Based Services and Children’s Waiver-Autism.
- Responsible for intake visits and paperwork associated with waiver programs.
- To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Current Basic Life Support (BLS) certification.
- Current automobile liability insurance in accordance with NTSOC policy.
- Knowledge of case management roles and responsibilities.
- Current knowledge of Medicaid and Medicaid Waiver programs
- Ability to relate to clients of all ages.
- Ability to complete Service Plans according to clients’ needs.
- Ability to communicate clearly and concisely.
- Empathy skills to effectively relate to clients’ needs and concerns.
- Travel distances to client’s homes for intakes and re-certifications annually.
- Attend community/marketing events as required.
