Job Description

Job Description

As an Outreach Specialist you will provide direct services to individuals with developmental disabilities living independently in transitional residential programs. Services include housing, support, transportation, didactic teaching, advocacy, service coordination, outreach, and assistance with community integration. You will work to develop individual service plans and help coordinate services between community, service providers and the individual

What you'll do:

  • Establish and maintain relationships with individuals and act as a liaison between the individual, community and other service providers
  • Assess and provide functional teaching in the areas of self-help skills, medication, activities of daily living, socialization, psychosocial need areas, leisure time, housing identification and maintenance, and community integration.
  • Ensure proper administration and documentation of all medication as needed.
  • Facilitate the assessment process and development of a service plan with the individual and others as appropriate.
  • Participate in the ISP process including note taking, ISP meetings, risk reviews and goal setting.

What you’ll need:

  • Education:
    • High School Diploma/GED equivalent required.
    • Bachelors Degree preferred
  • Experience:
    • Two to three years experience working with individuals with Developmental Disabilities.
  • Skills:
    • Must be MAP and CPR/First Aid certified within five months of hire (training provided)
    • Massachusetts Drivers License required
