Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals. This is a rewarding opportunity to make a positive impact in the lives of others.
Responsibilities:
- Conduct comprehensive assessments to determine client needs and develop individualized care plans
- Provide counseling and support services to clients, including crisis intervention and addiction counseling
- Collaborate with community resources and agencies to coordinate services and referrals
- Advocate for clients' rights and ensure they have access to necessary resources
- Monitor client progress and make appropriate adjustments to care plans as needed
- Provide ongoing mentoring and guidance to clients, promoting personal growth and self-sufficiency
- Maintain accurate and up-to-date case records
Requirements:
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Previous experience in child welfare, behavioral health, or related field
- Strong knowledge of crisis management techniques and intervention strategies
- Excellent communication skills, both verbal and written
- Ability to work independently and as part of a team
- Strong organizational skills with attention to detail
- Proficient in Microsoft Office Suite
Preferred Skills:
- Experience in addiction counseling or substance abuse treatment
- Knowledge of workforce development programs and resources
