CareSTL Health is Hiring a RN - Case Manager Near St. Louis, MO

CareSTL Health is seeking an experienced Nurse Case Manager. The Nurse Case Managers are registered nurses (RNs) who develop, implement, and evaluate individualized patient care plans. They act as social workers, advocate patient welfare, and serve as liaisons between patients, their families, and healthcare providers. Hospitals, hospices, nursing homes, home health agencies, and health insurance companies may employ RN case managers.

ESSENTIAL FUNCTIONS:

The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Nurse Case Manager (RN) may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.

  • Evaluate a patient to determine the level of care, patient, home planning other needs, and action plans
  • Complete expanded assessment of patient at the time of initial/new clinic encounter
  • Facilitate investigations, medical and treatment referrals
  • Works with agencies and service providers to facilitate case coordination and information sharing
  • Help patients condition options

Additional Duties

  • CCM/TCM and ER follow-up and management
  • Medication reconciliation
  • Interpret medical documents
  • Appropriate are appropriate and necessary
  • Coordinate the integration of social services/case management function
  • Evaluate patient satisfaction and quality of care provided
  • Communicate with physicians and develop a working relationship
  • Maintain appropriate cost
  • Refer cases where patients and/or family would benefit from counseling
  • Help patient and family make informed decisions
  • Review treatment goals and resources
  • Provide education and literature about diagnosis
  • Participate in the development and implementation of patient care policies and protocols
  • Promote effective and efficient utilization of clinical resources
  • Actively demonstrate a working knowledge of nursing theories, techniques, principles and

JOB REQUIREMENTS AND QUALIFICATIONS:


Education:

  • Graduate of an accredited School of Nursing
  • Valid registration as a Registered Professional Nurse in the State of Missouri
  • Current AHA Healthcare Provider training (BLS)

Experience:

  • 1-2yearsexperienceworkinginCommunityHealthCenter/or Hospital setting

Knowledge, Skills, and Abilities:

  • Knowledgeofinformationandtechniquesinprovidingexcellentpatientcare
  • Knowledgeofadministrativeandofficeproceduresandsystemssuchaselectronictransmission via facsimile and/or internet
  • Goodcomputerskillsincludingtheuseofofficeproceduresandterminology
  • Good self-motivational and organizational skills
  • Ability to maintain confidentiality
  • Ability to work all health center sites
  • Understanding of the complexities of integrating services
  • Knowledge of data management through use of IT

Transportation Requirement:

  • Must have reliable transportation

Supervisory Responsibility:

  • None

Additional Eligibility Qualifications (licenses, programs, certificates, pre-employment check, and post and annual screening(s)

  • Copy of Diploma or Official Transcripts
  • Current Registration with the Missouri Family Care Safety Registry
  • Annual PPD or Chest X-ray
  • Copy of Current CPR Certification
  • Current Nursing License for the State of Missouri

Training Requirements (licenses, programs, or certificates)

  • Graduate of an accredited School of Nursing
  • Valid registration as a Registered Professional Nurse in the State of Missouri
  • Current AHA Healthcare Provider training(BLS)

Additional Information:

  • Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
  • 11 paid holidays per year.
