Summary:
Clinical Case Managers are responsible for directing, coordinating and monitoring the treatment, utilization and general care of children within the residential program. Directs the treatment team in treatment planning. Facilitates intake and discharge planning with the ECH Screener, insurance companies, families, court representatives, and CD workers.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
· Completes admission process with the child, referral source, family/legal guardian on the day of placement
· Completes administrative functions associated with placement. Receives consents, collects documents and receives releases
· Completes Admission Assessment, Initial Service Plan and visitation and phone privileges
· Convenes and directs micro meetings and oversees service plan implementation
· Monitors and evaluates treatment team, child, family and legal guardians progress towards desired outcomes
· Schedules and directs Service Plan Reviews
· Communicates relevant information to all concerned parties between reviews
· Responsible for discharge planning
· Represents Agency and service team at judicial hearings, IEP meetings, psychiatric appointments, and other administrative meetings
· Primary contact for CD, DMH, DJO, and other managed care entities
· Primary contact for families and visiting resources. Develops and oversees visitation plans
· On Duty expectations—Due to the significant level of mental health issues experienced by our clients, a clinician is required to be available 24 hours a day to make therapeutic decisions to improve mental health status. Additionally, clinicians determine the need for psychiatric hospitalization when warranted
· Functions as a team leader
· Safe Crisis Management (SCM) competency
· Responsible for school registration, parent/teacher conferences
· Coordinates educational services with on-ground school staff, Normandy School staff, and/or Special School District school
· Complete progress notes in a timely manner
· Attend and complete trainings as established by ECH
· Other duties as assigned by supervisor
Education and/or Experience:
A Master’s degree in Counseling or Social Work (M.A. or MSW) or related field is required for this position. Preferred clinical practical experience.'
