Avenue360 Health and Wellness is Hiring a Director of Nursing/Quality (Heights) Near Houston, TX

Overview:

The Director of Nursing/Qualityprovides oversight of clinical activity and competency, staff supervision, as well as daily program monitoring of activities as instructed by the EVP, Clinical to ensure program integrity, performance improvement, compliance, and high quality of services. 

Participates as a key member of health center clinical leadership team in the development, implementation and evaluation of population health, quality, and practice transformation strategies.

Strategizes and drives process improvements focused on innovative care delivery and/or operational models designed to improve clinical services, outcomes, and patient safety.

Promotes the use and implementation of technology in the workplace to streamline operations, facilitate communications, and optimize work processes.In collaboration with leadership team members, directs and provides oversight regarding the strategic development, implementation, and maintenance of strong, comprehensive clinical and/or operational services and programs that meet system-wide organizational growth initiatives and the needs of patient populations in the community.

Meets established goals/benchmarks in the areas of patient safety, patient experience and clinical outcomes.

Advises the Chief ExecutiveOfficer/President, Executive Vice President - Clinical, and other members of leadership as appropriate on organization-wide, patient care programs.

Provides oversight and support for the Nursing teams, Health Information Technology Team, Health Information Management team, and Quality Team as well as affiliated clinical staff.

Leads organization-wide patient care programs and development of policies and procedures of patient care for nursing.

Approves and advises on nursing policies and procedures and nursing standards of practice.

Assists with development of retention programs for assigned employees.

Ensures that regulatory and accreditation requirements within scope of responsibility are met.

Works in conjunction with the Compliance and Operations team to plan for regulatory surveys.

Develops and maintains effective working relationships with external and internal customers to include patients, physicians, other managers and staff, referral sources, payers, auditors, contractors, and vendors.

Provides regular performance reviews, takes appropriate job actions, and reviews job actions taken bysubordinates to assure that staff meet qualification and performance standards and perform all essential functions of their respective jobs.

Utilizes knowledge of current trends and practices in the nursing profession.

With the EVP, Medical and the Avenue 360 directors, participate in the development, management and implementation of the development of QI and other health care policies, procedures and protocols for Avenue 360 programs and service lines while ensuring their consistency with applicable laws, regulations, program expectations, and professional standards

Applies population health concepts and implements appropriate quality improvement methodologies to assure highest standards of population health and quality improvement infrastructure and performance. Collaborates with the EVP, Medical to provide oversight of FTCA QA/QI compliance.

Oversee Avenue 360 data staff to enhance systems and tools (e.g. scorecards, dashboards) to collect, validate, analyze, report on key quality and performance metrics, and improve Avenue 360 clinical and operational data with respect to clinical reporting requirements and standards as established by funders and in alignment with Avenue 360 QAPI plan.

Convene and manage regular meetings and activities of the Avenue 360 QAPI committee under the direction of Avenue 360 EVP, Clinical.

May provides direct and ongoing outpatient clinical care for assigned patients and documents all client care in the EMR as appropriate in a timely manner.

Works closely with Vice President of Operations to ensure high quality of clinical care provided across agency

Other duties as assigned by EVP. 

Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements:

  • Masters in Nursing, Health Care Administration, Informatics or MBS required.

  • Current Registered Nurse license required.

  • Quality Management experience required.

  • Familiarity with ambulatory EMR systems (EPIC)required.

  • Basic Life Support (BLS) Certification

  • Experience with survey process (e.g. Joint Commission, FTCA, HRSA, etc.).

  • Must have strong organizational and project management skills.

  • Must have excellent judgment and creative problem-solving skills.

  • Highly organized and great attention to detail

  • Must have the ability to work with minimal supervision.

  • Must be able to work in a fast-paced, high-intensity work environment.

Continuing Education and Training Requirements:

CEUs required by Licensure and/or trainingsdesignated by the funding source.
