Salary Range: $78,000 to $94,000

About the role

The case manager will apply all nursing skills in this role, including infusion, wound care, ostomy teaching, medication teaching, and disease management. Our orientation program is based on your clinical experience and needs for further education. We encourage work-life balance through the ability to document at the point of care. We provide you with the tools to make this possible.

• Based on assessment, create, implement, and reassess the plan of care in conjunction with physician orders throughout the continuum of care while a patient is in service
• Coordinate patient care with other team members to obtain the best outcome based on patient/family needs.
• Provide hands-on nursing skills along with patient/family education per the patient's plan of care.
• Shift is 8 AM-5 PM, Monday-Friday
• Agency on-call requirements

Preferred Qualifications

• Home Health & Infusion
• 2-3 years in a range of nursing roles

Mandatory Qualifications

• Arizona Level One Fingerprint Clearance
• Current Drivers License, Car Insurance
• CPR
• Nursing License
• TB Test (provided)
