• Completes an initial assessment of patient and family to determine home care needs, including a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develop a care plan involving the patient and the family to establish goals based on nursing diagnosis incorporating therapeutic, preventative, and rehabilitative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the Provider (MD, DO, DPM, NP, and/or PA). 
• Provides direct patient care as defined in the State Nurse Practice Act.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
• Acts as Case Manager and assumes responsibility for coordinating patient care for assigned caseload.
• Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNA's and MSW's

• Associate's or Bachelor’s degree with one year of home health care experience preferred.
• AZ Nursing licensure in good standing.
• Current CPR/ BLS certification.
• Fingerprint clearance card.
• TB test results within the last year.
• AZ Driver's license and proof of insurance with reliable transportation
• Excellent observation, verbal and written communication and problem-solving skills
