Responsibilities:

- Conduct comprehensive assessments of client needs and develop individualized care plans

- Provide ongoing case management services to clients, including regular check-ins and monitoring progress

- Collaborate with other professionals, such as social workers and healthcare providers, to coordinate services and ensure continuity of care

- Assist clients in accessing community resources, such as childcare, behavioral health services, and workforce development programs

- Advocate for clients' rights and ensure they receive appropriate support and services

- Provide crisis intervention as needed to address immediate client needs

- Maintain accurate and up-to-date case records and documentation

Skills:

- Experience in behavioral health or social work field

- Ability to effectively communicate with diverse populations

- Proficient in conducting assessments and developing care plans

- Familiarity with motivational interviewing techniques

- Excellent organizational skills and attention to detail

- Ability to work independently and as part of a team
