Case Manager
Summary :
This is a contracted position, which provides case management services (CMS) to those who are reintegrating into the community from institutions, homelessness; and, who have complex health and/or behavioral health conditions and/or high utilizers of public services, including individuals with criminal justice histories and individuals who are exiting institutions such as hospitals, residential treatment programs, and
custody facilities.
Essential Duties and Responsibilities:
Provide outgoing outreach and engagement to the client population including field and
community-based locations, health and behavioral health facilities, interim and bridge housing
settings, criminal justice and custody facilities, and other locations as needed to engage thetarget population.
Establish a case management plan based on client’s authorization including but not limited to
establishing future goals, improvement of behaviors associated with drug use, reduction infrequency and quantity of drug and alcohol use, coping with mental health disorders, coping with chronic medical problems, improvement of interpersonal relationships.
Provide housing location services including assisting clients with locating affordable housing, providing assistance with negotiating rental agreements.
Assist clients with maintaining medication regimen.
Help accessing public benefits, educational, vocational, and employment services
opportunities as appropriate for each client.
On-going maintenance of program and client records; submit reports and invoices as needed and in a timely manner and provide all required supporting documentation.
Other duties may be assigned.
Education and/or Experience
experience working with individuals experiencing homelessness and past experience
working with clients with mental illness, chronic health issues, and substance use
disorders.

Physical Demands
While performing the duties of this job, the employee is required to stand and walk, regularly.
Employee must be able to climb or balance; stoop, kneel, crouch, or crawl. The employee must
be able to occasionally lift and/or move items weighing up to 10 pounds.
