We are currently looking for a motivated Full-Time RN Case Manager for our Home Health patients around the Metropolitan Phoenix Area and Scottsdale. For this Full-Time position, this RN will be paid per visit and incentivized to maintain higher visits per month with bonuses. Maintaining at least 25 visits a week will qualify this RN for full-time benefits including PTO, Health insurance, etc. The $60-65 is our starting per-visit rate and the hours per week is to mean visits per week (Indeed doesn't have per visit options).

Utilizing their depth of experience and knowledge, the home health nurse is responsible for:

• Completing comprehensive assessment of patients' who are bound to their home
• Conducting in home visits as assigned, assessing the patient’s condition, progressing towards goals, providing patient care, teaching and other skilled interventions as ordered by patients' PCPs
• Administering medications and treatments as prescribed by the physician

Requirements:

• Valid AZ RN license (or nurse compact license) (required)
• Home Health experience of 1-3 years (preferred)
• Experience with AXXESS EMR (preferred)
• Valid AZ Driver’s License and automobile insurance (required)
• Reliable transportation (required)
• Pass background check (required)
