Arizona Home Care located in Tempe is looking for a staffing coordinator to add to their team. The staffing coordinator will work in collaboration with the field clinicians and intake department to schedule all needs for patients. Position Summary:

Responsible for the accurate and timely scheduling of care for new patients for General Home Care (GHC) and Patient Care Infusion (PCI). Serves as a liaison between the field clinicians and patients. Works in collaboration within all departments of the agency to meet all patient needs. Promotes positive relationships with all internal and external customers. Compliant with all state, federal, and local regulations, and our accrediting body standards. Demonstrates well-defined organizational and time management skills. Observes safety guidelines and safe work practices.

Key Responsibilities:

• Responsible for the accurate and timely scheduling of care for patients.
• Evaluates the request for home care services for appropriateness.
• Provides additional information about the agency services to physicians, customers, and third-party payors as needed.
• Discharge or place on hold any patients who go off service. Updating this information in EMR and notifying appropriate employees.
• Confers with agency personnel on patient progress and ongoing evaluation and provides consultation on discharges for home care or readmissions to the facilities.
• Communication: proactive communication with subordinates, peers, and supervisory personnel is required at all times for all issues affecting job performance.
• May assist in training new staff on an as-needed basis.
• Flexible work schedule including Saturdays or Sundays, may be subject to change based on the company’s needs.
• Performs other duties as assigned.

Qualifications:

• Proficiency with computers, including data entry.
• Home Health experience is preferred but not required.
• High School education or equivalent required.
• Familiarity with Kan Time EMR but not required.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour
