Central Outreach Wellness Center is Hiring a Collaborative Care Manager Near Pittsburgh, PA

  • Provide support and coordination between patients, Primary Care Providers, and Psychiatric Consultant to provide patient-centered, trauma-informed, and culturally competent care in an LGBTQIA health center.
  • Work collaboratively with the mental health team and physical health providers.
  • Assist with communication between mental health and primary care teams to meet patient needs and optimize care.
  • Provide brief assessments and screening for common mental health and substance use disorders.
  • Engagement and rapport building with patients.
  • Systematically track patient treatment response to interventions and monitors patients for changes in clinical symptoms.
  • Communicate changes in clinical symptoms and treatment needs to the patients’ PCP and Psychiatric Consultant.
  • Use evidence-based brief interventions to assist patients in improving their mental health and wellbeing.
  • Refer to outside services, if indicated, such as case management or service coordination, drug & alcohol rehab, IOP or PHP, community therapists, support groups/meetings, clubhouses and drop-in centers.
  • Assess patient psychosocial needs and connect to resources.
  • Provide patient education about common physical, mental health, and substance use disorders and treatment options.
  • Participate in daily morning huddle.
  • Participate in regularly scheduled individual and group supervision.
  • Complete clinical documentation including communication with patients, treatment team providers, outside providers/resource, and patient-centered treatment plans.
  • Assess and document safety plans, if indicated.
  • Complete and submit monthly billing in the Electronic Medical Record.
  • Utilize techniques to re-engage patients who have dropped out of care.
  • Maintain and update community resources for external referrals and community services.

Licensure and Clearances:

  • MSW degree.
  • Successful passing of Child Abuse and FBI Clearances.


Requirements:

  • Minimum of 2 years of experience working in a Collaborative Care model.
  • Strong organizational skills.
  • Strong communication skills with both professionals and patients from all demographics.
  • Independently driven.
  • Demonstrated skill in managing difficult situations utilizing de-escalation techniques and conflict resolution skills.
  • Ability to work collaboratively in team-based care.
  • LGBTQ cultural competency.
  • Understanding of the relationship between physical health, mental health, and social determinants of health.
  • Commitment to providing high quality care to underserved populations.
  • Knowledge and comfort using evidence-based, time-sensitive, brief interventions, including Motivational Interviewing, Harm Reduction, Behavioral Activation, Brief CBT, and Problem-Solving Interventions.
  • Understanding on how to navigate medical systems.
  • Knowledge of basic computer skills, Excel, Word, and how to use Electronic Medical Records.
  • Need to be culturally competent while working , including being gender sensitive and pronoun aware
  • Punctuality and impeccable attendance

  • Patience and great attention to detail
  • Completely understand the healthcare privacy laws as outlined in HIPAA
  • Ability to work in a fast paced and, at times, excited atmosphere
