GENERAL RESPONSIBILITIES: The Case Manager provides comprehensive services to individual clients and their families. Services provided by case manager include assessment, case planning, developing individualized service plans/ treatment plans, care coordination, comprehensive transitional care from inpatient to other settings, referrals to information workshops, support groups and social support services. Reports to the Supervisor of Social Services.
