West Harlem Group Assistance, Inc.-West Harlem Residence is Hiring a Case Manager Near New York, NY

GENERAL RESPONSIBILITIES: The Case Manager provides comprehensive services to individual clients and their families. Services provided by case manager include assessment, case planning, developing individualized service plans/ treatment plans, care coordination, comprehensive transitional care from inpatient to other settings, referrals to information workshops, support groups and social support services. Reports to the Supervisor of Social Services.

Essential Functions:

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Interviews families to determine needs and appropriate services with the goal of restoring the family’s level of individual functioning as a unit.
  • Responsible for conducting initial intake interviews and assessments for all new clients and establishing a case file with all signed forms. Reviews the West Harlem Residence/DHS program-specific client policies and procedures with each new client.
  • Coordinates various aspects of Case Management services; including referral, intake, eligibility determination, program planning, monitoring, assessment, and evaluation of consumer needs / services.
  • Assists families in identifying the need for specific services and develops a mutually acceptable service plan. Ensures all service plans are developed, updated, and reviewed with clients according to program/regulatory mandates and client need(s).
  • Conducts weekly/bi-weekly’s with clients according to program/regulatory mandated frequencies. Documents all referrals for and contacts on behalf of clients. Provides information and referral services to clients as needed or as specified in the service plan.
  • Completes case management Via Cares database system. Responsible for collecting and collating monthly, quarterly, and as needed service statistics for his/her caseload.
  • Serves as a liaison with various agencies, including, but not limited to, schools, health care providers, mental health providers, and employment training providers in the interest of the clients.
  • Advocates for and assists families/individuals in the negotiation of the service delivery system, to ensure the receipt of entitlements and housing stabilization.
  • Provides Social Service Supervisor with an update of caseloads during weekly supervisions. Submit case notes and Service Plans for Supervisory review every Monday on a weekly basis.
  • Participates in weekly individual supervision with his/her supervisor, staff meetings, and team case conferences.
  • Provides evening and weekend coverage as assigned.
  • Provides emergency back-up coverage in the absence of the assigned Case Manager.

Required Education and Experience

  • Minimum three (3) years to seven (7) years of experience working in community organization, group work, community health or homeless services.
  • Excellent written and communication skills.
  • Ability to use computer to write reports, record data and analyze information.
  • Highly Preferred Education and Experience

BA/BS of Social Work Degree, or Human Services

Job Type: Full-time

Pay: Up to $25.07 per hour
