The Concierge ePrescribe Agent is a Pharmacy Technician-related position that assists the pharmacists with the mediation procurement process for our hospice partners all across the United States. Primary functions include providing provisionary assistance to clinicians for medication procurement. This also includes communication of refills to pharmacy partners, and verification of inventory at pharmacies for patient needs. Daily responsibilities are completed in an office setting utilizing a computer, working collectively as a team. Ability to work independently and multitask is also necessary for successful completion of daily responsibilities.
Essential Functions

1. Assists clinicians in the medication procurement process to include intake of medication requests from an agent of the prescriber, enter into e-prescribe system for physician to review, edit and electronically sign before being routed to appropriate pharmacy.
2. Completes verification process for quality patient care utilizing a 12-point pharmacy review to ensure prescriptions are accurately entered prior to sending to the physician for review and signature.
3. Assists in second check of peer’s work to ensure accuracy as necessary f
4. Navigates between various software systems for communication and completion of assignments.
5. Communicates both directly and indirectly within the organization and with hospice partners via chat, email, phone calls, and patient profile documentation.
6. Completes assigned responsibilities with a high level of efficiency and accuracy
7. Maintains a high level of productivity while balancing quality of care.
8. Answers the phone, responds to routine questions and outreaches to a pharmacist as needed.
9. Accurately includes necessary information when assisting clinicians for the procurement of hospice patient medication requirements.
10. Calls in patient prescriptions refills to designated dispensing pharmacies.
11. Outreaches to pharmacy partners to complete stock checks, ensuring medications are available for patient care.
12. Works cooperatively with fellow Concierge team members and others on various teams throughout the organization.
13. Assists in training new team members as assigned.
14. Adheres to all HIPAA protocols for patient privacy.
