Job Location: Main Street Care Center - Avon Lake, OH

Description

Qualifications: A high school diploma is required; an Associate's Degree is preferred, 1-2 years of experience in a related field in health care, office administration or assisting, must be proficient computer skills and working knowledge of general office machines, excellent grammar and punctuation skills, excellent customer service skills, must demonstrate independent initiative and be well organized, must have flexible schedule, must maintain the following qualities: team player, honest, dependable, trust-worthy and have excellent communication skills.
As an Administrative Therapy Assistant in a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to:

Essential Job Functions:
Assist with organizing therapy services, programs and activities.
Assist in transporting residents to and from the physical therapy room/area.
Reassure residents before and during therapy treatment.
Prepare resident for treatment by dress/position and administer physical therapy in accordance with established policies and procedures.
Assist with supervision of resident during therapy treatment.
Adapt to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and is flexible in carrying out assignments.
Communicate and interact professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control.
Communicate and interacts professionally and effectively with family members in attending to and meeting their requests.
Attend and participate in scheduled training, educational classes and meetings required or requested for position.
Ensure a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions and OSHA standards for healthcare professionals.
Respond to and act appropriately in emergency or disaster situations.
Adhere to standards of cleanliness, grooming, hygiene and dress code.
Maintain a current knowledge of the law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are in compliance with applicable regulations.
Maintain accurate telephone lists.
Create, maintains and assists in filling schedule as requested by the Director of Rehabilitation.
Operate the therapy computerized business programs.
Communicate effectively on telephone, via e-mail and in person.

Screen incoming calls and answer incoming correspondence as needed.
Report all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel
immediately.
Adhere to all policies of company, including job description, certification requirements, mission statement, HIPPA,
confidentiality standards and resident's rights.
Assist and completes patient scheduling for therapy services in coordination with nursing.
Responsible for contacting PRN staff to get appropriate staff coverage based on DOR's direction.
Pull all reports, including MD orders, delegated by DOR on a daily basis for morning meeting/DOR review.
Clean off ROX "hotlist" areas delegated by DOR on a daily basis.
Organize and tracks all anniversary dates/TB tests/therapy licenses and department compliance.
Enter all appropriate UR data from prior day and gives to DOR prior to MDS meeting.
Ensure that all correct/complete documents are in place at time of patient discharge and placed in medical chart per
SRHC protocol.
Complete technical portion of monthly therapy audit forms on timely basis for DOR review.
Perform other related duties as assigned and as required to effectively discharge the responsibilities of the position and
are in the best interests of the organization.

Individuals may be required to:
Sit or stand for extended periods of time
Reach above shoulder heights, below waist or lift as required to complete the job responsibilities
Proper lifting techniques, ability to read computer screens and the ability to follow written and verbal instructions are required
Must be able to lift 50 to 75 pounds. Raising objects from lower to a higher position or moving objects horizontally from
position to position. This requirement occurs to a considerable degree and requires the substantial use of the upper
extremities and back muscles.
Must be able to balance, climb, crouch, grasp, kneel, pull, push, reach, stand and stoop adequately to perform the
essential functions of the job.
Must be able to hear, smell, feel and speak adequately to perform the essential functions of the job.
Must be able to read and write the English language.
Must be able to work inside and outside with exposure to outdoor weather conditions.
