WORK SCHEDULE: Full time, Monday - Friday, 9:00 a.m. to 5:00 p.m. with some flexibility needed

SUMMARY:

Responsible for assisting clients with obtaining/maintaining entitlements and housing status as well as all stages of housing placements. Responsible for conducting home-visit assessments, intakes, and all enrollment related documentation. Responsible for providing direct services to clients. 

RESPONSIBILITIES:

• Secure entitlements for clients as needed, ensure maintenance of benefits.
• Provide program orientation to consumers.
• Conduct intake and assessment.
• Provide several client level services including supportive counseling individual/groups monthly.
• Monitor clients ability to comply with medication regimen and assist with adherence issues in conjunction with client's support services/providers. 
• Conduct follow-up visits to units on a regular basis.
• Connect clients to primary care, treatment adherence, harm reduction, mental health, and other supportive services to assist clients in maintaining HIV Care.
• Maintain a caseload of approx 13.
• Facilitate monthly house meetings collaboratively with other programs offered to residents
• Weekly input of client service delivery data and documentation into the relevant systems
• Participate as needed in weekly interdisciplinary case conferences to ensure continuity of service to all clients.
• Completion of safety and habitability inspections of all units
• Provide early Client Crisis Intervention as needed.
• Coordination with all providers to ensure wrap around services are in place

Qualifications

QUALIFICATIONS:

• BA/BS with some related experience, (will consider undergraduate internships or High School Diploma/GED with home finding or housing placement experience required) .

• Experience in a social service environment with target population preferred.

• Bilingual Spanish speaking preferred.

• Valid clean driver's license, with the ability to drive small passenger minivan preferred.

• Experience or knowledge of HRA 2010e application, Section 8, HUD, and HRA benefits preferred.

• Must be able to establish workload priorities and balance diverse duties.

• Must have excellent communication skills, both orally and in writing, with individuals and groups regarding complex and sensitive issues.
