About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Residential Group Home
- Patient Population: Adults
The Case Manager (RN) will:
- Coordinate the delivery of routine and specialty healthcare services to address assessed needs of persons receiving services in assigned program unit
- Assess and establish healthcare outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated
- Develops plan of nursing services and oversees the implementation of the health care plan by educating person’s receiving services, DSP/Caregivers and families in the plan of nursing services
- Educate, monitor and supervise staff in carrying out ordered treatment plan related to health needs
- Evaluate the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary
- Act as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers
- Complete and maintain all required written documentation on the approved electronic health record system
- Conduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members, as needed
- Complete relevant medical documents, MIPS and other required consent packets, surrogate packets, writing incident reports, death reports, assisting in developing policies, procedures and forms; and establishing working systems.
- Perform all other duties, as assigned.
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- Bachelor's Degree in Nursing
- Prior Nursing experience and/or experience working with persons with Developmental Disabilities
