Provides counseling and casework services to clients living with mental illness and substance abuse histories.
Responsibilities:
Meet with clients on caseload on a regular basis;
Coordination of entitlements, medical, psychological, recreation, substance abuse and transportation services;
Provide clinical interface with external service providers such as outpatient treatment programs, detoxification and rehabilitation programs;
Determine need for psychiatric referral and assessment;
Oversee clients’ compliance in taking prescribed medication and maintain medication documentation;
Maintain the charts of clients in accordance with agency requirements;
Participate in the ongoing discharge planning process, including both planned and unplanned discharges;
Participate in weekly Clinical Team Meetings.
Knowledge:
This position requires an Associates degree, BA/BSW preferred. Experience working with the MICA population preferred, some case management experience preferred. Prefer a minimum of two years experience working with the mentally ill and homeless populations, experience with substance abuse also preferred. Bilingual Spanish/English helpful. Must have exceptional communication skills both orally and in writing, with individuals and groups regarding confidential and sensitive issues. The physical ability to perform the essential job functions is needed. In addition, the Case Manager/Entitlements Specialist shall perform any duties as required to meet the needs of the program and the clients that we serve.
