Omni Community Health is looking for a trauma competent, quality-driven professional to join our amazing team as the Community Support Specialist (FITT - Family Intervention Treatment Team). As part of the Omni Family of Services, Omni Community Health is a behavioral health organization providing and coordinating care for children and adults. As the Community Support Specialist (CSS), you would be responsible for providing services to enable individuals and families to obtain access to needed medical, social, educational, and other support services as needed. The CSS function can be generally described as advocating, linking & coordinating needed community-based services for the client(s) and families served. The CSS provides a wide array of educational, role modeling, parenting, problem solving, and individual, group and family services using the Omni Community Health Core Strengths approach. This position will be a part of the FITT Program, which is an intensive, in-home program for youth and families. The Community Support Specialist is responsible for serving families with crisis diversion with skill building within the community and family home. Collaboration with team members, and on-call availability for crisis assistance and intervention is also required. The Community Support Specialist works closely with the assigned to provide needed assistance to children and families in their areas of expertise and reports to the FITT Director.
