I. TITLE:SERVICE & SUPPORT SPECIALIST

II. SUMMARY:
An employee in this classification, under the supervision of a Service & Support Administration Coordinator or designee, is responsible for supporting individuals with developmental disabilities in determining and pursuing life goals, and maintaining the individual focus and coordinating services and supports across multiple systems, while functioning as the Board’s primary point of coordination for each person on his/her caseload.
III. DUTIES:

Establishes an individual’s eligibility for services.
Provides service coordination to persons eligible.
Assesses individual needs for services, using a strengths-based approach, with consideration for important to/for, known/likely risks, working/not working, place on path to community employment, and integration in the community.
Develops Individual Service Plans (ISP), using Person Centered Planning, with the active participation of the individual to be served, other persons selected by the individual, and when applicable, the provider selected by the individual.
Recommends the plan for approval by the Ohio Department of Developmental Disabilities (DODD) when services included in the plans are funded through Medicaid Home and Community Based Services (HCBS) Waivers.
Establishes budgets for services based on the individual’s assessed needs and preferred ways of meeting those needs.
Assists individuals, through objective facilitation, in making selections from the providers and services available.
Ensures that services are effectively coordinated in order to meet individual’s needs.
Implements continuous quality review process of ISPs to ensure implementation of services and desired outcomes for the individual
Takes actions necessary to remediate any immediate concerns regarding individual health and safety.
Ensures that the results of provider compliance reviews and trends or patterns of unusual incidents and MUIs are addressed at the team review of an individual’s ISP for the purpose of improving and enhancing the quality and appropriateness of services rendered to the individual.
Connects individuals to community resources to meet assessed needs and/or to promote community life engagement.
Provides the individual receiving services an opportunity to designate the person to provide continuing representation, advocacy, advice, and assistance related to the day-to-day coordination of services in accordance with the individual service plan, and makes appropriate designations when the individuals served chooses not to.
Assures provider compliance with licensure rules as required.
Complies with all applicable rules, regulations, laws and Board policies and procedures.

Recommends policies and procedures for the operation of programs, and assures proper implementation in accordance with state and federal requirements.
Assists in development of assessment tools and related surveys for evaluation of services.
Participates in job related training and communicates with providers to maintain a current base of knowledge and expertise regarding residential services.
Consults with providers to improve quality of services, provides training and education, and may participate in provider reviews.
Serves on related committees as assigned.

May be required to transport individuals served.
Maintains a variety of records, reports, correspondence and other communication.
Attends a variety of staffings, meetings, conferences and activities.
May be required to orient individuals assigned to the department.
Performs other related duties as required.
V. EQUIPMENT OPERATED:
Personal computer, applications software, and basic office equipment.
VI. WORKING CONDITIONS:
Position may involve exposure to individuals with medical and behavioral risks. May require evenings and weekends, and locations convenient for person(s)/family(ies) served.
VII. KNOWLEDGE, SKILLS, AND ABILITIES:
Ability to coordinate services for diverse populations, cultures, religions, and ethnicities.
Knowledge of Person-Centered Planning, and Trauma Informed Care principles.
Public relations and communications skills.
Ability to establish and maintain harmonious relationships.
Knowledge of social work or related principles, practices and techniques.
Careful listening and counseling skills.
Computer skills related to spreadsheets, databases and word processing.
Knowledge of self-determination philosophy and principles.
Ability to facilitate exploration of individual choices, preferences, visions, aspirations and needs.
Knowledge of community resources, programs, services and supports for persons with dd.
Knowledge of local, state and federal laws, rules and regulations, and of the “Rights of Ohioans with Developmental Disabilities.”
Ability to promote an inclusive environment for all board staff, individuals served, families, and providers.
VIII. MINIMUM QUALIFICATIONS, EDUCATION AND EXPERIENCES:
A. Education and Experience:
Bachelor’s Degree in Social Work from an accredited program. Master’s degree preferred.
OR
Bachelor degree from an accredited college or university and one (1) year of experience in coordinating, assessing, and linking services and supports in addition to, or conjunction with, developing service plans for individuals. Service coordination experience with individuals with developmental disabilities or in a related field preferred. Master’s degree preferred.
OR
Bachelor degree from an accredited college or university and successful completion of a University or College sponsored one (1) academic year SSA internship with LCBDD. Master’s degree preferred.
B. Credentials:
Certification:Certification as issued by DODD
Licensure:Valid driver license
C. Ancillary Training:
CPR/AED and First Aid. Pediatric CPR will be required if caseload includes ages 0-12.
Training as required by the Board and DODD.
D. Other:
Requires reliable transportation
