- Provide face-to-face substance abuse psychosocial assessments
- Daily review and update of completing assessments, and other duties assigned by program director
- Facilitate patient referrals for additional services consistent with assessed needs and physician recommendation.
- Meet patients for follow up reviews or updates as needed or required
- Provide brief interventions, stabilization services and supportive counseling to patients
- Under direction of program director, care coordinator will provide patients medication education print outs that are focused on understanding illness, symptom management and medication compliance
- Complete and file documentation as required in timely manner
- Adheres to downtime procedures.
- Adheres to established departmental policies, procedures, and objectives.
- Performs both essential and non-essential job functions in a safe manner as identified by Humboldt Park Health.
- Demonstrates an understanding of and models the mission and core values of Humboldt Park Health.
- Enhances professional growth and development by accessing educational programs, job related literature, in-service meetings, and workshops/seminars.
- Checks hospital email daily.
- Performs other related duties as assigned.
