How you’ll serve our patients
The Care Coordinator plays a critical role in patient admissions and transitions, as well as providing administrative and clinical support to the treatment teams. They also ensure that all patients receive and complete treatment plan assessments and patient engagement surveys
You will serve patients daily in a multitude of ways:
Completes research assessments (TPA) and Patient Engagement Surveys (PES) with patients and family members throughout services
Work with Physicians and Clinical teams in various capacities such as scheduling rounds, patient appointments and D2Ds
Assist in admitting patients by communicating with patient and family prior to admission, completing and reviewing admission forms with them during the admission, including obtaining ROIs
Coordinate with outpatient providers by sending OP Provider Welcome Letter at admission and adding and updating contacts
Updating the electronic medical record with patient movements, transfers and discharges and involuntary status
Identify referrals for varying levels of care/treatment and coordinate with treatment team & providers to secure these services for patients
Work with treatment team members to facilitate FMLA/disability paperwork, obtaining work and school absence letters, etc
Must haves:
Bachelor’s degree in psychology, social work or related field preferred
2 years of relative experience in a clinical administrative setting preferred
