Position: Personal Care Assistant (PCA)
Responsibilities: • Acknowledges residents’ and families’ concerns and reports them to the nurse supervisor. • Responds to any and all call lights timely. Provides care or refers to appropriate person. • Provides privacy and maintains confidentiality. • Performs duties in accordance with accepted standards of resident care. • Participates in and receives nursing report upon reporting for duty. • Assists residents with resident care including bathing, grooming, hygiene and placement of adaptive equipment. • Explains procedures to residents before administering. • Keeps residents dry (changes gown, clothing, linen, etc., when it becomes wet or soiled). • Changes bed linens and keeps linens tight to avoid wrinkles from forming under residents. • Makes beds (occupied and unoccupied). • Handles linens according to infection control guidelines. • Assists residents with bowel and bladder functions. • Assists residents in preparation for and transporting to appointments. • Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs. • Assists residents in ambulation with or without self-help devices as instructed.
• Accurately measures heights and weights.
• Accurately measures temperatures, pulses, respirations and blood pressures (TPRs and B/P) as instructed.
• Empties urinary drainage bags and reports amount to nurse supervisor as indicated.
• Ensures call lights are within reach of residents and answers call lights promptly
• Ensures that residents who are unable to call for help are checked frequently.
• Ensures that residents’ personal care needs are being met in accordance with residents’ wishes.
• Reports all changes in residents’ condition to licensed nurse supervisor as soon as practical.
• Cleans and disinfects reusable equipment and returns to proper area after use.
• Straightens dresser tops and stands.
• Maintains cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms and linen closets; cleans bedpans, urinals and commodes; washes basins and emesis basins; sanitizes bath tub after each resident’s use; cleans floors if resident is incontinent; wipes spilled foods and liquids.
• Provides post-mortem care as instructed.
Special Care Responsibilities:
• Identifies and reports abnormal skin conditions.
• Turns bedfast residents as instructed.
• Provides Range of Motion exercises.
• Assists with application of adaptive devices.
• Provides daily indwelling catheter care.
• Provides perineal care.
• Assists with personal engagement activities as time permits.
Food Service Responsibilities:
• Assures that residents are positioned appropriately for meals.
• Assures that appropriate utensils are within reach of residents.
• Serves food trays and assists with feeding as indicated.
• Keeps residents’ water pitchers clean and filled with fresh ice and water (on each shift) and within easy reach of residents.
• Delivers snacks and supplements.
• Provides hydration to all residents according to center policy.
Administrative Responsibilities:
• Reports observations concerning structural, equipment, and furniture defects and malfunctioning to appropriate personnel.
• Maintains confidentiality of necessary information.
• Utilizes supplies and equipment properly and without waste.
• Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride.
• Performs any miscellaneous work assignments as may be required.
