Position Summary
This Case Manager works across the Health Services Division; CRW clinic and West Village Houses, delivering case management services to a clientele ranging from 18 to 100 years old to assist in accessing and navigating the systems that impact the social determinants of health such as employment, economic, education, health, housing and social supports. This is a full-time (35hrs) case management staff position that offers flexible scheduling Monday through Friday –9am to 7pm. Services are delivered in person. The case manager will empower clients to lead enriched lives as they cope with issues throughout the life cycle including but not limited to mental health, medical health, substance use recovery and the aging process.

We will consider exceptions for religious and medical reasons, where appropriate. “Fully vaccinated” means you must have both doses of a 2-dose vaccine and two weeks have elapsed since the second dose or have received 1 dose of a 1-dose vaccine and two weeks have elapsed since the dose. Proof of vaccination status must be submitted prior to your start date and includes CDC Vaccine Card, NYS Excelsior Pass, NYC Vaccination Record, and other official Vaccine Record.
