Position Summary 

This Case Manager works across the Health Services Division; CRW clinic and West Village Houses, delivering case management services to a clientele ranging from 18 to 100 years old to assist in accessing and navigating the systems that impact the social determinants of health such as employment, economic, education, health, housing and social supports. This is a full-time (35hrs) case management staff position that offers flexible scheduling Monday through Friday –9am to 7pm. Services are delivered in person. The case manager will empower clients to lead enriched lives as they cope with issues throughout the life cycle including but not limited to mental health, medical health, substance use recovery and the aging process.

Primary Responsibilities:

• Provide direct case management services for application/renewal of benefits and entitlements. Refer clients to appropriate community resources, e.g., securing identification, employment, legal, housing, accessing healthcare services, life planning and assisting with applications and documentation.
• Collaborate with Clinicians and Home Health Aides on shared clients.
• Collaborate with referring external partners on shared clients.
• Timely completion of documentation in the electronic health record and other required reports to comply with: DFTA; NYS-OMH regulations and requirements to include but not limited to incident reports, progress notes: and encounter forms. 
• Continuously identify untapped resources for populations served.
• Report to supervisor about client’s unmet needs.
• Serve on the agency’s committees as assigned.
• Immediate notification of Directors and appropriate clinical staff of reportable incidents and at-risk behaviors. 
• Perform other tasks and special projects as assigned by the direct supervisor.
• Participate in weekly supervision and interdisciplinary staff meetings.

Qualifications:

• MSW or related social service degree with 1-3 three years case management or social work experience.
• Experience in trauma informed care preferred.
• Fluent with benefits and entitlements
• Strong computer skills and data entry skills needed.
• Must comply with the agency’s standard of ethics, conduct and organization policies.
• Work collaboratively with interdisciplinary team to accomplish program goals and adapt to the changing program environment.
• Must be able to establish priorities and a course of action for handling multiple tasks.
• Fluent in English and Spanish

We will consider exceptions for religious and medical reasons, where appropriate. “Fully vaccinated” means you must have both doses of a 2-dose vaccine and two weeks have elapsed since the second dose or have received 1 dose of a 1-dose vaccine and two weeks have elapsed since the dose. Proof of vaccination status must be submitted prior to your start date and includes CDC Vaccine Card, NYS Excelsior Pass, NYC Vaccination Record, and other official Vaccine Record.
