Job Description

The Program Coordinator has the primary responsibility for providing leadership, management, regulatory compliance, supervision, and program plan implementation to our home caregiver and 245D Community Based Services and programs. The Program Coordinator will provide direct care to people we serve during scheduled shifts, manage the staff at the agency, coordinate meetings, and establish relationships with the Interdisciplinary Teams, functioning as a liaison between the people we serve, teams and the agency. The Program Coordinator is responsible for being available during emergencies.

Essential Functions

Duties would include but not limited to:

  • Lead intake and required home visit meetings with participants, families and caregivers
  • Develops and implements the care/support plans for all individuals within the program
  • Educate participants and caregivers on the PCA program, HCBS programs, 245D, rules and regulations
  • Train, orient, evaluate staff, and oversee month-by-month plan of services
  • Responsibly maintain required program documentation, assist with caregivers supervision as requested by DHS regulations and the participant/family, and oversee the delivery of PCA services.
  • Maintain excellent communication with participants, families, counties and state agencies
  • Manage on-going activities of daily living, encouraging independent living skills whenever possible
  • Accurately and thoroughly document day-to-day client progress on scheduled programs and record all data requested on the appropriate program charts; supervise staff in the completion of daily progress tracking
  • Generates and supports all aspects of 245D services which includes CSSP Addendums, Program and Individual Abuse Plans, and carry out behavior management procedures as outlined in individual behavior plans
  • Obtain a working knowledge of client history, skills, areas of vulnerability, habilitation plans, and emergency procedures
  • Advocate for client’s needs; encourage and provide opportunities for client choice, presenting creative options as needed
  • Perform site assessments and quality assurance visits to clients’ homes to assure complete satisfaction of care services
  • Communicate all performance concerns, educational needs and development opportunities to the management in a timely manner.
  • Develop and maintain a filing system, update and maintain office policies and procedures
  • Analyzes market conditions and strategically improves the company's market position for financial growth.
  • Calling on and developing new referral accounts in the local healthcare and senior care community with elder care industry professionals, organizations and establishments (ex. Geriatric Care Managers, independent/assisted/skilled living facilities, hospitals, concierge doctors, etc)
  • Creates and implements a business development strategy to identify new client opportunities.
  • Researches the competition and generates potential leads.
  • Gathers information on potential clients and builds client relationships.
  • Builds partnerships with other companies.
  • Attending local community events and even hosting events to increase brand awareness
  • Finds ways to better serve existing markets and creates marketing campaigns.
  • Seeks out and develops relationships with various referral sources.
  • Establishes a networking plan and actively uses it as a working tool, which is revisited as requested/needed.
  • Visits existing referral sources to maintain and grow partner relationships.
  • Updates prospects, clients, and referral sources database for contact/and or mailing lists.
  • Tracks networking efforts and reviews them with supervisor on a weekly basis.
  • May perform initial evaluation of services for potential clients.
  • Perform site assessments and quality assurance visits to clients’ homes to assure complete satisfaction of care services

Qualifications

Applicants must be able to meet the criteria for a Qualified Developmental Disability Professional (QDDP) and/or Designated Coordinator as assigned by 245D which includes a combination of work experience and education.

  • BA/BS in Human Services or related field with 1 year of full-time (or equivalent) work experience providing direct care services to persons with disabilities or persons age 65 years and older OR
  • Associate degree in a human services or related field: 2 years of full-time (or equivalent) work experience providing direct care services to persons with disabilities or persons age 65 years and older OR
  • Diploma in a human service related field from an accredited post-secondary institute, 3 years of full time (or equivalent) work experience providing direct care services to persons with disabilities or persons age 65 years and older OR
  • A minimum of 4 years’ experience providing direct care services to people with disabilities under the supervision of someone who meets the above criteria.
  • Experienced with IHS, ICLS, and other 245D services.
