JOB SUMMARY

Works collaboratively and interdepartmentally to provide holistic care via care coordination, education, pre-visit planning, and connection to internal and external support services for patients in the MAT program. Manage day to day patient flow in the medical clinic and responsible for coordinating activities such as documenting care plans, medication reconciliation, recall to follow-up clinical quality improvement and health care home.

ESSENTIAL FUNCTIONS:

  • Enroll patients into the onsite IHB MAT program, scheduling and completing MAT intakes according to IHB protocols.
  • Manage day to day patient flow within the clinic for patients enrolled in MAT program.
  • Create and monitor internal patient registry for timely and proper follow up per provider recommendations and IHB policies.
  • Monitor Providers allotted DEA prescribing data to ensure providers stay within prescribing range.
  • Update information in IHB's DEA folder to ensure compliance with DEA standards for the MAT program.
  • Lead monthly MAT team meetings and prepare report to provide updates on patients enrolled in MAT program.
  • Act as Health Care Home Care Coordinator, seeking to understand and meet the needs of the patient through respectful and caring interactions with patients, their families and other health professionals.
  • Serve as a linkage between patients and their internal and external healthcare and service providers to improve coordination of services and information flow to ensure effective treatment plans.
  • Works with the patient, providers, and Health Care Home Care Team members to develop and maintain an individualized clinical Care Plan for the patient, reviewing regularly and updating as needed, per IHB's Health Care Home policies.
  • Assists patients with the identification, selection, monitoring and documentation of self-management goals, monitoring the patient's progress toward goal achievement and modifying as needed.
  • Utilize information from psychosocial assessments with patients and serve as an informed advocate for the patients and recipients of family planning services to understand needs (i.e., shelter; transportation, child care, safety)
  • Work with patients to access other specialty providers as needed.
  • Performs general ambulatory health care to patients, including administration and/or dispensing of prescribed medication and treatments in accordance with clinic policies and procedures.
  • Provide patient triage in person or via phone as part of the medical care team.
  • Perform clinical assessments of patients within scope of practice and needs of the position.
  • Assist with Hepatitis C referrals, follow up, and provide patient education
  • Ensure effective communication between patient entry to care and care team (i.e. effective communication and teamwork between front desk/nursing/providers)
  • Identify and report clinical, utilization and outcomes issues. Identify workflow issues and recommendations for improvement.
  • Lead projects to improve outcomes and quality measures, ensuring compliance with nursing quality activities
  • Maintains and implements department and organizational policies and procedures
  • Maintains nursing standards, policies and procedures manuals and workflows.
  • Manage the medication management standards within IHB to maintain compliance with Joint Commission including sample medications, dispensed medications, and onsite administered medications.
  • Actively participate in MAT / Recovery Services planning in medical clinic.
  • Provide excellent customer service to patients, peers, leadership, and community partners.
  • Maintain patient information in the EHR.
  • Represent the Indian Health Board at meetings and with other organizations as appropriate.
  • Other duties as assigned by supervisor.

EXPERIENCE & QUALIFICATIONS

  • Registered Nurse (R.N.) licensed in the State of Minnesota
  • 2 years of experience in clinical operations that include MAT Services preferred
  • Experience leading quality improvement initiatives
  • Knowledge of Medical Home Model of care
  • Experience in care coordination services
  • Skill in establishing and maintaining effective working relationship with patients, medical staff and the public.
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Excellent communication skills in a medical setting, with patients, the medical team and community partners.
  • Self-motivated with effective critical thinking skills.
