ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Takes a lead role in coordinating and providing the team’s medication administration services

Provides education to clients, families and other ACT team members about mental illness, physical health, medications, and side effects

Takes a lead role in the direction and coordination in the physical health treatment as stated in the treatment plan.

~ Collaborating with client’s physical health care providers

~ Will accompany ACT client’s medical appointments, if necessary, and facilitate medical follow-up

Conducts any needed health assessments for psychiatrist—blood pressure, weights, and temperatures.

Completes any other needed health assessments which are appropriate for the LPN scope of practice

Works with ACT clients and staff developing and implementing person-centered treatment plans which include goals addressing physical health issues.

Assures appropriate, effective service delivery.

Must attend all mandated meetings conducted by the Bureau (DHH) or its designee.

Participates in the ongoing assessment and treatment engagement of ACT clients.

Completes, as directed, elements of the comprehensive assessment for clients within 60 days of admission and coordinates information with the team to effectively plan treatment.

Transports and/or accompanies clients to appointments related to community services and resources.

Locates and/or develops self-help and support groups for clients, families and significant others.

Attends ACT Team meetings to create an integrated, a multidisciplinary team approach to services:

~ Daily Team Meetings- to organize work for each day and review recent contacts with each client.

~ Treatment Planning Meeting- weekly or as needed to develop and review treatment plans/decisions

~ Clinical Supervision Meeting- 1-2 times monthly for training, case consultation and professional development of ACT staff

Maintains compliance with all applicable practice standards and guidelines.

Maintains client confidentiality and adherence to HIPAA requirements.

Completes all required documentation in a timely manner consistent with agency guidelines.

Maintains agency productivity standards.

With documented consent, engages the client’s family and/or significant others to provide education regarding mental illness and community resources.

Adheres to the policies, procedures, and work rules set by the Agency.
