Albany County Department of Mental Health is seeking applications to fill a vacancy for a Health Home Care Manager to work with individuals with Behavioral Health Challenges. This is a competitive civil service position that may be filled provisionally by the appointment of a person who meets the minimum qualifications established for such positions. Hours will be discussed further if an interview is offered.

Full Performance Knowledge, Skills, Abilities and Personal Characteristics

Thorough knowledge of persons with mental illness and substance abuse disorders diagnoses. Good Knowledge of the Health Home Care Management program. Ability to write clear and concise care management plans. Working knowledge of the public mental health system. Ability to provide services in the community

Minimum Qualifications:

Must be a New York State Licensed Master Social Worker/Licensed Clinical Social Worker, Licensed Mental Health Counselor, Registered Professional Nurse and have two years of professional experience providing direct services to persons diagnosed with mental disabilities or linking persons diagnosed with mental disabilities to services essential to living in a community setting;

OR,

Graduate from a regionally accredited or New York State registered college or university with a Master’s degree in Forensic Mental Health and two(2) years of experience providing direct services to persons diagnosed with mental disabilities or linking persons diagnosed with mental disabilities to services essential to living in a community setting,

OR,

Graduation from a regionally accredited or New York State registered college or university with a Bachelor’s degree and five(5) years’ experience providing direct services to persons diagnosed with mental disabilities or linking persons diagnosed with mental disabilities to services essential to living in a community setting.
