HEALTH DEPARTMENT OF is Hiring a Health Care Surveyor 3 (Nursing) - 46790 Near Albany, NY

Permanent Transfer Candidates: Current NYS employee with one year of permanent or contingent permanent competitive service as a Health Care Surveyor 3, G22, or Community Nursing Services Consultant Home Health Services, G22 and eligible for transfer under Section 70.1 of the Civil Service Law OR Current NYS employee with one year of permanent or contingent permanent competitive service in a title G20 or higher and meeting the open competitive minimum qualifications listed below.
Open Competitive: possession of a license and current registration to practice nursing in New York State; and three years of post-licensure, specialized professional,
administrative or professional clinical experience.
Preferred Qualifications: Excellent leadership skills and ability to lead and participate on an interdisciplinary team. Excellent interview, observational, and writing skills, and an ability to translate collected information into legally defensible reports. Experience with the aged and aging population is key, especially in the provision of direct patient care as a human services or clinical professional, and an ability to appreciate how the aging process affects outcomes is paramount to the incumbent's success. Flexibility and dedication are fundamental to the team's ability to address the needs associated with surveillance.

Duties Description

The incumbent functions as a member of an interdisciplinary surveillance team that can include other nurses, social workers, registered dietitians, and sanitarians. The incumbent performs at an advanced level and may serve as a team leader. In addition, the incumbent serves as a nursing services expert on surveillance teams, and may lead, coordinate and oversee the activities of lower level staff. The incumbent provides leadership and direction in the performance of the following duties: Conduct the survey/investigative process in a thorough, accurate, and timely manner that is consistent with Department policies and procedures, ensuring that survey assignments are completed in an accurate, comprehensive, and efficient manner; Act a team leader of a survey team and provide guidance, training, and direction to lower grade survey staff; Document findings of survey investigations in a clear, well organized, objective, and relevant manner to support surveillance/enforcement actions, ensuring that reports are well written, focused on substantive elements of an issue, and are factually supported; Review and make recommendations regarding physician’s orders, pharmacy communications and medical care provided to long-term care residents; Monitor correction of deficiencies and identify areas of substandard performance within area of medical care and make recommendations for follow-up action; Act as liaison with the supervisor/program manager regarding problems encountered in the surveillance/ investigative process; Demonstrate professional competence and responsibility in the performance of duties by conducting surveys/investigations in a tactful and professional manner, with minimal disruption to facility staff; Demonstrate sound professional knowledge as well as thorough knowledge of applicable regulations, interpreting policies/regulations accurately and clearly; Communicate findings/significant issues clearly, appropriately, and promptly to facility staff and program managers; Demonstrate professional discretion in communicating with facilities and with other surveyors, working independently or in consort with other professional staff; Maintain a current knowledge base within the scope of clinical expertise, Department policy, and state and federal regulations, cooperating and participating in program modifications and assignments; Represent the Area Office and Department in contacts with providers, the public, and other agencies as required and/or directed in conformance with Department policy by responding to requests in a professional, knowledgeable, courteous, and timely manner; Prepare correspondence in a concise, comprehensive, and appropriate manner in accordance with required timeframes; Review pertinent aspects of an issue and demonstrate an ability to articulate significant issues in a focused manner that encompasses an understanding of regulations, Department policy, and professional knowledge; Prepare for and participate in enforcement proceedings by providing accurate, reliable, objective, and responsive testimony that supports Department findings by (a) Identifying systems problems, giving scope and examples in a comprehensive, clear, and objective manner, and (b) demonstrating professional knowledge and sound personal judgment in the conduct of survey activities and in the formulation of any resulting deficiency citations that are well developed and expressed in a manner that accurately conveys the noted problems; Report issues that need direct input and may impact on region, Division, and Department policy; Respond to public health emergencies and participate in preparedness related activities, training and exercises as directed and needed; and other duties as assigned.

Additional Comments
  • Posted salary range includes the statutory hiring rate at step 4 and job rate for PS&T Grade 22 ($85,272 - $95,392) and an additional Geographic Pay Differential of $20,000
Work hours and telecommuting availability (50% maximum) for this position will be discussed during the interview process, and are subject to operational needs and the telecommuting application process.
