We are searching for an LPN-Float Health Care Coordinator to join our agency. This position contributes to the Mission of the Center, the Nursing Department, and Residential Program by providing and overseeing the medical management for the people we support assigned to his/her caseload. Provides direct oversight for health and medical issues of the people we support. Maintains regulatory compliance of required documents. Provides oversight for direct care staff, regarding topics related to and associated with maintenance of the health and safety of the people we support. The work schedule for this position is Sunday-Thursday 12PM-8PM.

• NYS LPN License required
• High School Diploma or equivalent 
• At least 2 years of related experience required, 2 years of related experience working in the field of Intellectual and Developmental Disabilities preferred
