How You Will Make a Difference

As we expand our services in new and existing markets, we're looking for a Director, VitalCaring Connection (Telehealth Services) who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, called to serve. The Director, VitalCaring Connection (Telehealth Services) serves as the subject matter expert to advance home health services through the implementation of technology and virtual care strategies. In this role, you are responsible for developing, overseeing, and optimizing virtual care programs aimed at enhancing patient care, improving outcomes, utilization management and expanding access to healthcare services.

As the Director, VitalCaring Connection (Telehealth Services), you will:

• Develop a comprehensive virtual care program aligned with the company's mission, incorporating innovative technologies and practices to improve patient care in a home health setting
• Lead the implementation and integration of virtual care initiatives, including telehealth platforms, remote monitoring devices, and other digital health solutions, ensuring seamless integration with existing home health services
• Build and manage a skilled team of virtual care professionals, providing guidance, support, and mentorship to ensure high-quality service delivery and optimal patient outcomes
• Collaborate with internal stakeholders, healthcare providers, technology vendors, and external partners to foster relationships, and identify opportunities for collaboration in delivering virtual care services
• Establish and monitor key performance indicators (KPIs) to assess the effectiveness of virtual care programs, continuously evaluating and improving services to maintain high standards of care

Skills for Success:

• Knowledge of Medicare guidelines and the “Conditions of Participation” for Home Health
• Ability to analyze the documentation required to support homebound status for patients
• Identify the ongoing need for intermediate skilled nursing, physical therapy, speech therapy, or the continuation of occupational therapy

Experience to Deliver on our Mission:

• Bachelor’s degree in healthcare administration, nursing, business, or a related field
• Valid state driver’s license and auto liability insurance
• Minimum of three (3) years’ experience in home health and/or hospice
