Position Summary:
Collaborates with medical staff, unit managers, and all professional care giver disciplines to plan and coordinate the plan of care with patients and families throughout the acute continuum of care and into post acute settings. Utilizes resources to maximize the health care benefits to patients in a cost-effective manner. Assists in guiding quality care in accordance with best practice guidelines and protocols by communicating effectively with physicians. Responds to Director/Manager as required. Works well in a fast paced environment and demonstrates effective critical thinking and decision making.
This position would be 2- 8 hour shifts per week.
Position Qualification:
RN with preferred BSN
Hospital experience >3 years with CM experience (prefer hospital CM experience)
Prefer Cerner experience
Flexible & adaptable for prn position
Ability to multi task, problem solve & use critical thinking skills.
Strong communication skills
