Description

Overview: The Care Coordinator helps adolescents and young adults with significant behavioral health and medical needs to optimize their outcomes through the coordination of their care activities. Working both onsite and offsite in a manner that is responsive to the unique needs of Door members, the Care Coordinator conducts outreach, assessment, treatment planning, referrals, and coordination of service activities for a small caseload of individuals. This is done through collaborative setting and achievement of person-centered goals with clients, and the organization of care activities so that the various providers in a client’s orbit are working in collaboration with each other. Ongoing training and professional supervision are provided.

Responsibilities:

  • Conduct regular outreach to young people
  • Thoroughly assess each young person and develop service plans based on those assessments
  • Ensure that clients receive referrals to appropriate Door and community resources
  • Facilitate service delivery through work conducted both on and offsite as needed
  • Coordinate treatment plans with various service providers
  • Maintain all reporting requirements, including those of the agency, Health Homes, and all regulatory authorities
  • Work as an active member of The Door’s Behavior Management Team (BMT) and attend all associated meetings
  • Coordinate and facilitate case conferences, including staff from all program areas/agencies to determine appropriate plans for young people
  • Provide de-escalation and crisis intervention as necessary
  • Participate in agency-wide and department staff meetings monthly, as well as all scheduled team meetings
  • Complete any additional responsibilities and functions as assigned by supervisor

Qualifications:

  • BA in related field and four years of relevant professional experience, preferably in a mental health program; or a Master’s degree in related field with two years relevant professional experience
  • Demonstrated ability to engage hard-to-reach populations
  • Familiarity and experience with trauma-informed approaches to service coordination and delivery
  • Experience and/or knowledge working with street-involved youth
  • Knowledgeable of and ability to apply positive youth development principles to working with young people
  • Understanding of adolescent development, assessment, counseling, and/or crisis management
  • Excellent oral and written communication skills required
  • Proficiency in Spanish, French or Mandarin strongly preferred
  • Ability to work effectively and creatively as part of an interdisciplinary team

Work Schedule: Monday - Friday, 35 hours a week including afternoon and evening hours
