We are currently seeking a Care Coordinator in our 400 Broadway location.

Position summary: Lead a team of patient navigators and patient assistant to ensure care coordination services are provided to all clients assigned to meet clients needs for better health outcome. Provide day-to-day supervision to the patient navigators and patient assistant assigned by weekly schedule monitoring, chart review, face-to-face case discussion, and performance reviews. Responsible for implementation of service/care plan, supported by Patient Navigators. Conduct outreach & recruitment for Apicha CHC to promote the Agency services.

Qualifications: BA/BS in related field with two ( 2) years of qualifying experiences required . An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience may be considered; Experience working with target populations strongly preferred. The target populations: HIV/AIDS who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis; Ability to adapt changing environment; Appreciation of and commitment to Apichas mission and values required; Excellent MS Office skills (Outlook, Word, Excel, PowerPoint) required; Ability to work effectively in both a collaborative as well as individual setting; Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC; Excellent customer service skills to assist in resolving member complaints; Strong verbal and written communication skills for both internal and external audience.
