We are currently working with a longstanding community-based organization in New York City, that caters to the needs of children, youth, families, homeless individuals, and older adults across Manhattan and the city, striving for a fair and equitable society where everyone can pursue better lives.

The Intensive Mobile Treatment (IMT) Team offers specialized treatment and support services to individuals who frequently engage with mental health, substance abuse, criminal justice, and homeless services systems, often underserved by conventional treatment approaches. IMT teams maintain continuous contact with individuals, irrespective of their location, whether in shelters, on the streets, in housing, hospitals, or jails.

Purpose of Position: The Registered Nurse (RN) assumes responsibility for providing direct and comprehensive nursing care to clients of the IMT team, including oversight of medication management. The RN's role encompasses enhancing clients' overall health, with a focus on promoting the integration of physical and mental health. Operating in alignment with principles of recovery-oriented and trauma-informed service delivery, the RN contributes to fostering holistic wellness among clients.

Responsibilities:

• Medication Management
  1. Coordinate with team psychiatrist/PNP regarding client prescriptions
  2. Supervise, dispense, and document client medications
  3. Serve as a liaison between psychiatrist/PNP and pharmacy
  4. Manage storage and documentation of onsite client prescriptions and medications.
• Nursing Duties
  1. Evaluate the psychiatric and medical conditions of clients
  2. Conduct medical assessments and health screenings
  3. Facilitate referrals to primary and specialized healthcare
  4. Educate clients on health promotion and wellness
  5. Conduct blood draws
  6. Administer and monitor psychotropic injections in accordance with client preferences and professional standards
  7. Conduct visits to clients in residential facilities, on the streets, and other settings
  8. Maintain accurate and current client case records as per program guidelines
• IMT Team Contributor Duties
  1. Attend team meetings, case conferences, and training sessions to enhance skills and knowledge
  2. Engage in supervision meetings to assess client progress and refine case management approaches
  3. Collaborate effectively with IMT team colleagues within a team-based case management framework

Requirements:

• Active NY State License as a Registered Professional Nurse 
• 2- 3 years of nursing experience preferred
• Essential experience with individuals coping with substance abuse, mental health challenges, homelessness, and trauma
• Preference for prior involvement with IMT Team
• Proficiency in Spanish is advantageous
• Computer Skills: To excel in this role, proficiency is required in the Microsoft Office Suite, particularly Word, Outlook, and Excel
• Capability to utilize or adapt to the AWARDS database
• Aptitude to employ or acquire proficiency in various electronic databases, applications, and funder-specific software like NIMRS and Maven
