Founded in 1974, Community Access is a pioneer of supportive housing and social services in NYC for people with mental health concerns. Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, East Village Access—a PROS program, Peer-driven Crisis Respite Center, Adult Home Initiative and Assertive Community Treatment (ACT).
Department Overview
Community Access provides transitional andpermanent affordable supportive housing to individuals with psychiatric disabilities and low-income community residents. Our transitional housing model helps people with psychiatric disabilities move from shelters and hospitals into the community. All transitional housing sites provide on-site staffing 24 hours a day as well as individual counseling, goal planning and linkage to outside programs and services. The focus of the work is to help people regain basic skills such as negotiating the public transportation system, cooking, shopping, maintaining personal space and hygiene, budgeting and organizing their time around activities they enjoy, and help them adjust to a community setting.

Wage: $98,654.40 – $105,232.40

Position Overview
The Registered Nurse works with the program staff to promote the medical and psychiatric health of tenants. This collaborative process has as its goals assisting tenants practice behaviors that promote wellness; helping tenants assume responsibility for their own health; encouraging periodic assessments and examinations that support early intervention when illness occurs; supporting treatment for chronic conditions that help tenants lead their lives as fully as possible. This includes assisting with the coordination of monitoring medication services in accordance with regulatory requirements and good practice. Some additional responsibilities of the Registered Nurse are providing overall assessments of program participants’ health concerns including psychiatric, medical and substance abuse status. The Registered Nurse is responsible to undertake health assessments, including screening for metabolic syndrome, diabetes and hypertension and assist with monitoring and coordination of program participants’ medical, psychiatric, and substance use treatment, in conjunction with various internal and external service providers.
